Families of SMA Canada

To make a donation to the Families of SMA Canada Society, you may:

• Call the office with credit card information

• Download a pledge form and mail or fax it with cheque or credit card information

• Donate to FSMAC using CanadaHelps (secure online credit card transaction processing)

If you wish your donation to be in honor or memory of someone, please let us know that so that we can notify the correct family of your donation. Please have the address available if possible.
