To share your story on the FSMA Canada website (see
the Photo Album), please print the photo
release form, fill it out and send it (with your photo or
photos) to:

Alternatively, you can scan the completed form
and email it to

along with your photos and a story.
When you share your photos for our online photo
album, they also become part of the collection of photos for fundraising
event displays. We have many requests for photos, as this is a very
effective way to tell our stories. It is also be helpful if you include some basic
biographical details. The bio can be simple if you like - name, type of
SMA, date of birth (and passing if applicable), likes, something to let people
know this person is a unique individual - or it can be a longer story.
If you ever want your page updated, please simply email the update and
new photos bettylou@curesma.ca.
